Alterations / Amendments in the Passports

An application with the relevant documents must be submitted to this Mission in order to process the subsequent amendments/alternations as follows;
 
Compulsory documents
  • Duly filled application (download)
  • Valid Sri Lankan passport
  • Letter of Request
In addition to the compulsory documents, it is necessary to submit the additional documents required under the following circumstances.
 
For change of name in the passport after marriage (in addition to the compulsory documents)
If the applicant is married and wishes to have the spouse's surname in the passport;
  • Original Marriage Certificate (Marriage certificates issued by a Country/State other than Sri Lanka should be authenticated by the State Government in the relevant country of origin. Then the certified documents must be certified by the accredited Diplomatic Mission of Sri Lanka in the relevant country)
 
For all other name changes in the passport (in addition to the compulsory documents)
If the applicant wishes to change the original name indicated in the previous passport relevant to the original Sri Lankan birth certificate
  • Original Sri Lankan Birth Certificate (name change must be stated in 13th column of the birth certificate)
 
Include / Change of profession in the passport (in addition to the compulsory documents)
If the profession needs to be included/changed in the passport;
  • Degree Certificate / Higher educational qualification / Employment verification letter / Documents to prove the profession (Certificates issued by a Country/State other than Sri Lanka should be authenticated by the State Government in the relevant country of origin. Then the certified documents must be certified by the accredited Diplomatic Mission of Sri Lanka in the relevant country)
 
The Prescribed Fee
  • Endorsement on alteration page USD 12.00
 
Payment Options
Payments should be done at this Mission by cash / money orders / bank drafts /cheques.
 
Money orders / Certified cheques should be made payable to the “Permanent Mission of Sri Lanka to the United Nations, New York”.
 
Card payments are not acceptable.

Note:
  • Applicants are strictly required to present the originals of all the documents required with the application. Translated copies of any documents are not acceptable.
  • Submission of forged documents and making false declarations relating to citizenship is a punishable offense. Fees will not be refunded in these circumstances.
  • Incomplete applications will be rejected.

Return of the documents
Applicants will be informed once the passport is ready for collection. He/she is required to bring the cash receipt issued in order to collect the passport.
 
If the applicant is unable to collect the passport personally he/she should send a nominated person with a letter of authorization to collect the passport on behalf of the applicant. The nominated person should provide the authorization letter along with his / her photo ID, the applicant's current passport, cash receipt.
 
At request of the applicant, the new passport will be returned by post.
 
If the return of the documents is desired by post, applicants in the United States may choose one of the following options and make payment in addition to the application fee:
 
  • Domestic Mailing Fee - Federal Express (FedEx)                – USD 20.00 (within U.S.A.)
  • International Mailing Fee - Federal Express (FedEx)           – Relevant Country Rate
 
This Mission is not responsible for any documents lost in the post.
