Registration of Deaths

Death of Sri Lankan nationals may be registered with this Mission and an application for this purpose may be made by a nearest relative of the deceased or any person who was either present at the time of the death or in attendance during the last illness of the deceased or who took action to dispose the body of the deceased by burial, cremation on by any other means.
 
The declaration should be submitted within 03 months of such death to this Mission. For registrations after 03 months a letter of explanation with the reason for the delay must provide with the application.
 
The applications made after 12 months from the date of the death, will be forwarded to the Registrar General’s Department in Colombo 10, Sri Lanka (RGD)  for approval.
 
Compulsory documents
Applicants are needed to present originals of all the relevant documents required on submission of the application.
 
Application and the necessary documents must be filled and submitted by the declarant, a close relative to the deceased who was present at the time of death, in attendance during the last illness of the deceased or who took action to dispose of the body of the deceased by burial, cremation on by other means.
 
  • Duly filled declaration form (download) – for registration within 03 months
  • Duly filled declaration form (download) – for registration after 03 months
  • Following documents of the deceased must be submitted by the declarant
    • Original Death Certificate issued by the relevant authority
    • If the human remains have been repatriated to Sri Lanka for burial/cremation, the original of the Airway Bill (Air Cargo Bill).  If the human remains have been buried/cremated in the United States, a report issued by the relevant Authority.
    • Coroner’s Report / Autopsy report
    • Original Sri Lankan Birth Certificate
    • Current Sri Lankan Passport
  • Documents to prove the relationship between the deceased and the declarant
    • Original Marriage Certificate of the declarant – (if the deceased is the spouse/child)
    • Original Birth Certificate of the declarant – (if the deceased is the parent)
    • Original Passport of the declarant
  • Letter of explanation with the reason for late registration (registration after 03 months)
 
Note:-
All applications submitted after 12 months will be forwarded to the Registrar General's Department (RGD) in Colombo 10, Sri Lanka for consent /approval. In such cases, the death certificate will be issued only after approval is granted by the Registrar-General.
 
The prescribed fee
  • Registration within 12 months      – USD 13.00
 
Payment Options
Payments should be done at this Mission by cash/money orders/bank drafts /cheques.
Money orders / Certified cheques should be made payable to the “Permanent Mission of Sri Lanka to the United Nations, New York”.
 
Card payments are not acceptable.

The duly filled applications can submit to the consular division of this Mission by post or by hand.

Note:-
  • Applicants are strictly required to present the originals of the documents required with the application. Translated copies of any document are not acceptable.
  • Submission of forged documents and making false declarations relating to citizenship is a punishable offense. Fees will not be refunded in these circumstances.
  • Incomplete applications will be rejected.

Processing Time

  • Registration within 12 months     – 05-07 working days
  • Registration after 12 months       – Upon approval of RGD
 
Return of the documents
Certificate of death will be issued only to the declarant.
 
If the declarant is desired the return of the documents by post, may choose one of the following options and make payment added to the application fee:
 
  • Domestic Mailing Fee - Federal Express (FedEx)                – USD 20.00 (within U.S.)
  • International Mailing Fee - Federal Express (FedEx)           – Relevant Country Rate
This Mission is not responsible for any documents lost in the post.
